Change the 6-row to 2-row. No need for the extra enzymes in 6-row if you're only mashing a bit of crystal.

I think you may need a larger pot or lower the amount of base grain. With the water numbers you have you're looking to get ~4.5 gallons of runnings. I know that if I put 4 gallons into my 5 gallon pot I'd definetly have a boil-over.
 
I think chinook and cascade are a good combo. From the maltiness chart (see this link:
) I think you're in the very hoppy range. On top of the strong ABV, this will be an only-having-one-or-two kind of brew. Personally I'd try to drop out about 10 IBU. Maybe eliminate the 60 minute 1oz of cascades. And/Or take 1/4 oz or so of your chinook and divide it into a couple of later flavor/aroma additions. Chinook is good for more than just bittering, just go light and combine it w/Cascade.
