How high to fill bottles

Sorry for another noob question, but how high should a 12oz bottle be filled? Does there need to be room for CO2 to expand into while fermenting further? Will filling too full create too much pressure?

General rule of thumb: Not less than 1/2 inch and not more than 2 inches. I use a bottle filler, and on a standard bottle, it is pretty close 1.25 inches of headspace. It is less on stubbies and more on pints (due to the amount of volume being taken up by the bottle filler).
 
With my bottle filler - the cheapy plastic one that most HBS's have - I fill all the way to the top and then upon removal of the filler the level drops to leave the appropriate head space.
 
Just agreeing with what Ian said. Withdrawing the filler tube leaves about 1.5" of headspace.
