Cord to elements, 3 or 4 wire?

If I have one of the following:

  1. 3 wire dryer cord from my dryer outlet to GFCI spa panel and 4 wire cord from spa panel to controller box
  2. 30A GFCI breaker in my main panel with 4 wire line running to an outlet to which I plug in my controller box with a 4 wire cord
Do I need 3 wire cords or 4 wire cords to go from my controller to an element in order to properly ground everything (including my BK)?
 
You only need three wires to your kettle. the two hot lines and the ground. the neutral line does not connect to the kettle.
