The obvious answer is no. This is my process
Mash at X temp for Y minutes
Drain wort into kettle
Add sparge water to tun and mix
Drain wort into kettle.

I ask because this is how i thought it was done but according to John Palmer when you mash, before you drain any wort, you add your sparge water then drain.

As for efficiency, I am usually 70-80 with my current method.
 
I'm pretty sure your referring to batch sparging that's the way I do all grain too if I'm understanding you right. Say you mash @150 for and hour than drain the mt and add more water to sparge the grain at around 170 and then drain and combine the two in the kettle. There's a good calculator to help get your temps right at tasty brew.
