My problem with pouring from larger bottles

When I pour my homebrew from bottles that are larger than 12oz, the sediment gets stirred up quite a lot after the first pour. My first pour from the bottle is very clear but after setting the bottle back upright, the second pour is quite cloudy and yeasty. This always happens even with long aged bottles. Anyone else have this problem?
 
Happens to me, too. Sometimes I'll pour two glass in one tilt, especially if it's heavily carbonated and I run out of room.
 
Happens to me as well. I'm not crazy about larger than 12oz bottles except for swing tops for obvious reasons. When I do pour them I'm like evilgnome and tend to pour all of it at once.
