I have some 1st year cascades, around 15 feet. they have lots of cones on them and are starting to send side shoots from the bottom 2-3 feet of the bine.

should I trim those off, twist them on the bine, or start a new one....

I trained my first side shoots then got tired of messing with all the buggers! The rest found their own way up by wrapping around other bines\sideshoots. I say let them go, especially since they are first year. If they dangle they will just end up finding something to grow up. So if you have other plants or anything close, I would train it where you want it before it finds something on its own.

I train the bottom side arms up onto the main bine or support rope just to keep them off the ground. All the others I let go crazy unless they are about to grab a neighbor of a different variety.
