First time using a liquid yeast. Using Wyeast activator pouch.
Didn't notice that I had to refridgerate that pouch upon arrival. I've had it for 3-4 weeks in ~60º F. I activated the pack last night and wondering if I should use this yeast to brew today. Anyone have any advice for me?
The pouch is partially swelled. Do I chance this or buy more?

The yeast is fine. That being said it is highly recommended to make an appropriate stater for all liquid yeast. Go to Www.yeastcalc.com to figure out the size of the starter required for your beer
