Alphabetical order, most important to me are quality of selection, variety, innovation, availability and price:

Great Lakes (I'm from Cleveland)

Brooklyn may be on it's way out of the top 5 depending on how I enjoy some other breweries.
 
This should be a top 5 list because I could make a good list that would just be annoying. I'm from California and we have THE BEST! I'll even try to provide my favorite beers one that is a special brew and one that is not a "specialty" beer.
1) Firestone Walker- Velvet Merkin, Double Jack
2) Sierra Nevada- Porter, Estate
3) Lagunitas- Little Sumptin' Sumptin' Wild, Little Sumptin' Sumptin'
4) Bear Republic- Hop Rod, Red Rocket
5) Stone- Ruination, Oaked Double Bastard
 
I think alot of this depends on where you live. I live in SW Canada and my favorites are:

Driftwood Brewery (Victoria BC)
Ninkasi (Eugene OR)
Parallel 49 (Vancouver BC)
Sierra Nevada (Chico CA)
Deschutes (Bend OR)
 
I'm from Farmington, nm in northwest NM. So I have to rep some of the local!

Ska
Three Rivers
Marble
Steamworks

And non local has to be

Rogue, Ballast Point, Lagunitas.
 
No particular order

I'm from WV, SN is the only one on the list available here. Gotta cross state lines to get anything really good.
