Well people it finally happened to me....I have gushers....not 100% sure if it's an infection or just inconsistent carbonation(I'm leaning heavily toward infection).
How does one prevent this infection?...I am anal as the next guy when it comes to santitation and still this infection found its way into my bottles.

Any and all input would be greatly appreciated.

Is there a ring around the neck? If so, definite infection.

I used to have a gusher problem, then I followed the Papazian recommendation of soaking all my bottles (I use Grolsch-type) in a bleach solution (1 cup in 5 gallons) overnight, scrubbing them with a bottle brush and a hot water rinse, sanitizing and then filling! It worked! I am currently enjoying my batch of British Bitter and it's infection free!
 
How long in the bottle.

Did it taste fine.

How long in fridge.

Ived had perfectly good beer appear to be a gusher only to be fine a few days or weeks later.
