Please help me add yeast to bottle an 11% ABV RIS

I brewed this on March 30 this year. I racked to secondary on May 14. It has been sitting since. I have not taken a gravity reading, because I don't want to mess with it until I'm ready to bottle.
According to BeerSmith, I want to add 2.5oz. of corn sugar to the beer before bottling.
I'm pretty sure I will need to add more yeast also. I used US-05 for fermentation, and that is what I have on hand now. I'm just trying to figure out how much to add to my bottling bucket. I figure I should hydrate it, pour it in and stir it.
