One of two reasons most likely. Your priming sugar wasn't mixed evenly in solution or your sanitation on some of your bottles was poor.
 
Thank you for the advise. I'm going to examine my process. I'm thinking it may be because I'm using my washed yeast from previous brews.
 
Would that be caused by sanitation? I'm very careful to wash everything well and soak it with STARSAN. Would it be caused from washed yeast that was possibly contaminated. Any idea how I can determine the cause and correct it?
I really appreciate your help. I always consider Revvys words as absolute truth.
