So I bought a rootbeer extract and dry wine yeast from the local shop that sells brew supplies. This was all based on their recommendation. They also suggested waiting 2 weeks. I followed the instructions but didn't get a lot of carbonation. I got a little, just not much.

I noticed that the top of the 2 liter soda bottle had warped. Is this why it may have failed? Has anyone used Dry Wine Yeast to make root beer?
 
I also used .5 1/8 teaspoon of the dry wine yeast. Should I have put in more yeast?

That should be plenty. It does sound like a bad seal. It shouldn't take 2 weeks to carbonate - in warm weather my bottles get rock hard in about 3 days, maybe a week at room temp in the winter. Then into the fridge - and you'll get better carbonation if it's been in the fridge for a 3-4 days before you crack it open.
