Most folks 'round here will recommend making a starter with any liquid yeasts... even the smack pack variety. I do the same.
 
Depends on your OG ! If it's higher than 1.065 I would make a starter, otherwise, the vial is made to pitch into 5 gals. I always make a starter if I have a few day's prior to brew day, if I don't then I go by the og. I still think the vial is underpitching somewhat. Cheers
 
make a starter, you want fermentation to start ASAP. The longer it takes for fermentation to start, the more chance there is of getting an infection.
 
I usually make three gallon batches and will do a starter unless it is a low O.G. (1.040 or less) and the yeast is only 2-3 weeks old.
