Does anyone have an opinion on this it is something I put together with some leftovers I have

Looks like a simple, moderate to aggressively bitter APA. OG will come in around 1.050 to 1.058 depending on volumes and efficiency. Your hop schedule gives about 50 IBUs. Personal taste, but I would move the 40 to be a 10 minute addition, and move the 5 to flame out, dropping about 10 IBU in the process but bringing out more of the Willamette floral aroma and flavor. But then again I'm not you! :) I love making recipes by using up what's laying around. Best wishes...
 
I would use a little less crystal,and lower IBUs with less hops IMHO.

I agree nearly 30% crystal is a bit excessive, but no one is going to die from it.
 
What about changing the 40 and 5 minute hop additions to .5 Oz this would reduce the ibu to about 37-37

That looks better balanced. Any of the suggestions can make fine beer though. I like willamettes and hence my desire to max out the late additions. I'd give it a shot, post how it turned out in a couple months.
