High gravity..

I'm trying to learn from my mistakes with my first all grain. My OG should have been 1.040. It ended up being 1.060. What could have been the reason for such a high gravity?

1lb of minute rice.
I ended up adding almost 2lbs of minute rice by mistake.
I did BierMunchers Cream ale recipe. I ended up with 7.5 gallons pre boil. I hope you can help me why I was so off. I'm doing another batch this weekend.
 
At 75% efficiency the OG should be around 1.057 so you were close.

What efficiency and volume does BM have on his recipe?
 
Even if you left the rice out completely and had 70% the gravity would still be over 1.040. Are you sure the 1.040 wasn't the pre-boil gravity on his recipe?
 
Assuming that you stuck to the recipe with 1 lb of rice and 75% efficiency, a pre boil volume of 6.5 gallons would have a gravity of about 1.040 and after the boil have a volume of 5 gallons at 1.051. With your extra rice and probably some oversparging, that's how you got that high.
