For a 12 oz, non stubby bottle I do a height of 3.5 inches and from 3-7 inches wide, depending on design.
 
What do you use to design them? I tried powerpoint, but I can't seem to get 4 labels on one page and full size (3.5x5)
 
I used the labels you can buy at any local office supply store they are 3-1/3"x4"
Avery 5164 or even 5664 (they are removable)

I did my artwork in illustrator and then used the word doc template for the labels to place them correctly
 
My labels are 4" tall by 4.25" wide, but they're just a hair too tall for non-stubby bottles. I'm probably going to shorten them a little bit.

EDIT: I use Inkscape to design my labels. Free open-source vector graphics program. It takes a bit to learn the interface, but it's more intuitive than, say, GIMP.
