Question on the small Neck-Label

So I'm designing my label for the first beer.

And I want to add a small band-label on the neck of the bottle. The label is can't be a straight rectangle, since the bottle is curved. Has anyone worked up a shape that works well with this bottle:

Here's what I use as a template....

You have to resize it depending upon the dot pitch of your printer and with a little trial and error.

Ah, thanks. I had the idea to scan one of the ones that came off the bottle I'm using, but I'll play around with this too.
