Kegged beer is pretty flat

Assuming no leaks, what would be a reason for flat beer from keg? Going on 4 weeks now in the kegerator at 12 psi. I checked the gauge with friends new regulator and also assume that's accurate. Using 10 foot line. There's some head but just tastes really flat. Frustrating.
 
Temperature?

My hoses are about 7 ft for 12 psi at 48 - 50 degrees.
 
Hose length should only be an issue for foaming during serving and not for carbonating. Seems you would need to be below 40F to carb at 12psi (I don't have a chart handy). Try increasing the pressure to 25psi for 24 hours and see if things improve.
