sensory evaluation kit?

Does anyone know where I could find a sensory evaluation kit. I know I've seen one for sale on line that came with examples of all off flavors and info on each, I just can't for the life of me remember where it was.

Here's a link to a document I put together to do some flawed beer tastings for my club. This is just info that I pulled together and compiled off of a few different websites, but it provides a decent starting place using things that are pretty common.
