I'm assuming you want to add dry malt extract to make up for a lack of efficiency in an ag batch. If that's the case, you'd add it to the boil. You need to be more specific in your question.

I ferment my lagers at about 45 or so (depending upon strain) until about 75% complete, then I move to room temp for about 3-4...

Sounds like you didn't have the regulator completely sealed to the tank. Time to get the proper tools to seat the regulator to the tank. You might want to purchase some teflon tape too.
