Did you let it sit to carbonate in the bottle? If not it may have been the yeast that you used. If it was a beer yeast strain it may have reached it's ABV limit in primary.

The honey will still carb at the same rate, the reason you are using so much honey is to backsweeten the cider as well. That is why the recipe suggests pasteurising the cider to prevent bottle bombs...

Sounds fine to me. You might have a slight leak between the carboy and the airlock. Give it a gravity reading if you are unsure. You shouldn't use airlock activity as a sign of fermentation.

Just a heads up, if it is mold the batch is pretty much ruined. Racking under the mold won't work as the mold itself releases toxins into the cider that can't be avoided.
