While fermentation should be done in 5-7 days I feel it is necessary to let it sit for 4 weeks. There's some clean up going on that seems to make a difference for me.

For the diacetyl rest you need to bring the temps up to 65F for a few days. You should be fermenting around 50F. So no, you cannot have both at the same temps at different stages of fermentation.

Conditioning and carbing are different animals. Carbing at 65F is fine but if you are looking to lager\condition further then as close to 32F as you can get would be optimal.
