I would use Simcoe at 60 and I LOVE Citra at 15. That amount of Cascade probably wont make much difference wherever you put it.

To get 13 gallons into my BK I usually end up batch sparging twice...

Aside from posting the wrong strain (it was 2035)...

Aside from posting the wrong strain (it was 2035) its a lager fermentation due to the use of a lager yeast within its temperature tolerance.

Do you have sediment or haze? If it is indeed calcium deposits and nothing else works, you could try a liquid coffee cleaner like Java Kleen and see if that helps.
