pardon me for adding to the thousand posts on proper concentrations for sanitizer, but I've always sort of glanced around the boards because I routinely forget the amounts required to mix up a batch...

Hello,

So due to poor planning I activated a smack pack and about a week went by without it being used, a day or two at the beginning spent at room temperature before I put it back in the fridge....

Did you use the little packet of yeast that came with the kit?
If so, it's possible that the packet was just super old and the yeast was barely viable. How strong was the fermentation in the primary?
