It shouldn't take that long. I do the set and forget method and the beer is typically ok at 8-10 days but nicely carbed around 14-16 days.

I would spray everything down with star san or soapy...

Try using the hydrometer for FG, make sure it's calibrated correctly and the sample is at the calibrated temp. I believe the refractometer can be skewed because of the alcohol in the FG sample.
