Open it when you are ready to pitch. I suspect the yeast nutrients were released and the yeast is munching on it. You should be able to pitch with no problems.

The gas does not fit on the beer side. The gas is slightly smaller than the beer side. If you get it on, it may be very difficult to remove it. (I know)

Have you done the leak test? Spray soapy water or starsan on all the connections to see where the leak is coming from? It produces bubbles.

I'd recommend that after drinking a bottle, rinse heavily. Wash with a bottle brush as soon as you can with soap, PBW, etc. Then store in a dry location. Sanitize before filling.
