Give it a couple more weeks in the bottle. The half bottle had too much head space - how long did you cool it down before you tried it? Probably didn't get the CO2 into suspension.

SB - if your water is good to drink, you can top off with it without boiling it. Been doing it for 20+ years all over the US without an issue ever. Beer is tougher than most here give it credit for....
