    The recipe doesn't look bad to me, what does your water profile look like? My initial thought
    is that all of the dark/crystal malt brought your mash pH down too low.

    Looks good, almost all of your flavor/aroma comes from late additions/dry hop. So I would say a
    30/45 addition is not needed as your ibu's look to be in line for a moderately hoppy beer.

    Just as a note, I'm pretty sure Alpine uses Weyermann CaraAmber and not Amber malt in Duet as I
    have never seen a sack of Amber malt in the brewery. CaraAmber however is all over the place.
