Sounds like you might need a new regulator. If you set your regulator to 12psi but it got up to

You have a leak in your system somewhere that you need to find. A 5 pound tank will last you a
dozen kegs or more if you're not leaking gas.

The AHS site also says to add it in with the grains during mashing. I think I am going to give
it a shot that way. Adding it to the boil sounds like it could get a lot messier.
