If you think the first starter has fermented out I'd cold crash the first starter, decant and
step it up with the new packet of yeast. My 2 cents.

Just to be sure its not a temperature issue, pour some into a glass and take a temp reading.
Make sure the probe only touches the beer and not any part of the glass. Since you say your IPA
pours fine ...

Calibrate your thermometer. You could be mashing at a lower temperature than you think. I would
not add more crystal malt and mash higher; do one at a time to help rule out any issues.
