Default Totally missed my OG. What happened?

Followed this recipe for the extract version:

OG came out to 1.040 and a abv of 5%. First time Ive been off like this, but also the first time I did a 90min boil. How could I have undershot it by so much?

Could be wort that's not mixed or a hydrometer that's not calibrated.

Good call on the hydrometer. I didnt calibrate for temperature. Thanks.
