Default Brews gone BAD!

So I was thinking after this weekend's batch that may have potentially gone bad, and maybe the mods can help out with this as as a sticky.

Even with extract brewing there are plenty of steps where mistakes can be made, it would be nice to be able, with the resources available here, to put together a troubleshooting guide of sorts.

Such as if you get this result, this is most likely your problem, and it can be resolved like this.

If this happens, this is probably what happened and you are out of luck.

I briefly remember reading about a plastic taste that was a result of infection.

but some of this things that come to mind are what happens to brew with infections and the best possible actions to take.

what about using different water, hard, soft, spring and what the brewing results that would be.

Pitching yeast in too warm a temp, or too cold a temp.

While I know the answers are out there in various threads it would be nice to put them together in one!

thoughts? additional questions or common problems that most brewers run into?

Most people don't inquire about issues until it happens to them, it would be nice to put this together and read this BEFORE it happens to know WHAT is happening as it is happening?

Sorry for the thought jumping post, I wanted to put this out there before my 8am meeting!
