Default Low gravity reading

So i just brewed a pale with a friend who was interested in the process. He picked the BB micro pale kit and i suggested 1/2# crystal 40, 1# DME, and 1oz. Simcoe in addition. We steeped, then added one 3.3 muntons light and the 1# DME. Started hop additions and 30 minutes later added the other can of LME.
When we checked the SG is was only 1.046. That is the lowest reading I have ever seen with this much extract. Does a late addition change gravity reading? I would not think so. Took the reading right at 60F. Any comments on the subject would be great. Thanks.

Did you adjust hydro reading for temp?

If yes than it may just be the extract is not mixed totally.
