Been pulling crappy efficiencies
Not just crappy because they are low, but completely unpredictable. I'll use identical methods and they'll range from 58 to 72. How am I supposed to design recipes if I have no idea what kind of efficiency I'll land.

Just venting. Time to RDWHAHB.

I'm the same way - and although my methods are the "same" each time; my execution of said methods is not.

For example, I often miss my mash temps by 1-3* either way (high or low), I often sparge 3-5*F cooler than planned, I don't always drain my MLT completely until the last sparge, I boil-off different amounts, etc. etc.....

sounds like you can plan for ~65% using your system and be pretty close if you hit 58% or 72%

Here is Kaiser's Troubleshooting Brewhouse Efficiency paper, it should help. IMO it's a worthwhile exercise to do his Efficiency-Troubleshooting-Spreadsheet (linked near the bottom of that paper)...that should help you see exactly why your efficiency is so unpredictable.
