Default Uneven Carbonation Question

So here is my dilemma .
I have a very nice pumpkin ale that has been bottled for 3 months now. Over the course of that 3 months I've opened six of them. Three of them have been totally flat, one borderline "ready", one "perfect" and one was over carbonated. Those have come in random order, no relationship to time in the bottle.
This is a high gravity beer (OG 1.100, FG 1.024), Wyeast 1318 yeast.
I'm assuming I got poor priming sugar distribution and this is causing the uneven carbonation.
My question(s).
Is there a way to tell by looking through the bottle if a beer is carbed?
Is my best option, to have carb tabs and steralized caps ready whenever I open one of these beers? If I don't get a good hiss on opening, drop in a couple carb tabs and re-seal? Or is it possible, even with poor priming sugar distribution, that a bottle will carb over time in a high gravity beer?

Storage temperatures have been roughly 65-70F
