Default priming with DME

What is the ratio of DME to volume, and dissolve it in 2+/- cups of water?

1) Quick method is 1.25 cups for a five gallon batch.
2) To carbonate by style.
3) A cup or two of water should be fine. It will need to be boiled and cooled.
