perhaps http://www.midwestsupplies.com/catalogsearch/result/?q=screw+cap may work. They have a threaded bottle with two types of closures.
The problem is that I don't think those caps or threads are like the ones you find commercially. Those tend to be a non-threaded sheath that drops down around the bottle and are then formed by rollers around the threads of the bottle. By this process, you may not be able to find a machine that does this. Unless you have access to a company or college that does.
