Default Extremely high starting gravity?

My roommate and I brewed a batch of wheat ale the other day and ended up with a starting gravity significantly above what was on the recipe. We thought we did everything by the book, but aren't sure after seeing that.

Thoughts?

What was the recipe? Original gravity? How did you brew it?
