Default 3 way Co2 Distributer -Can I make it 4?

I bought a It is sold as a 3 way but there is a capped end with threads? Is this simply a hollowed out aluminum block? Can I take capped end and add a valve?

Yes. Take the plug out of the end and add another valve.

If it is a primary reg, I thought they are left hand threads, on the sides.

I did this with my 3 way manifold. The thread at the end was 3/8 NPT so I needed to get a bushing. Couldn't find one so I used a 3/8 nipple and then a 3/8 to 1/4" reducing coupling.
