Default Sparging

So I did my 1st AG last night and have a few questions.

I am using a cooler mash tun, do you stir the mash?

When fly sparging should there be a solid grain bed with water above it slowly passing through?

i usually stir my mash halfway through. seems to raise efficiency 5% or so

Same here, stir halfway. I batch sparge, but I believe the technique is to have at least an inch or two of water above the grain bed when fly sparging

I stir the initial mash until the temperature is stable and where I want it. After that, I leave the mash to, well, mash. It doesn't get stirred again until it's drained and I've added the batch sparge water.

Depends on your sparge method...I fly sparge and don't stir at all

Mashing I stir once when adding grain. Then when I do a two step batch sparge, I add my first half of water, stir like it owes me a cupcake, then the same for the second batch of water.

If you are going to stir, do it thirty minutes before vorlauf and let things settle back down. I agree with progmac, it probably adds efficiency. Otherwise, it's going to make vorlauf longer to get to clear runnings if you stir too close to vorlauf time. I've always stirred with thirty minutes left in mash whether fly sparging or batch sparging (1st batch). Depending on the type of mash tun, it also brings more consistent mash temp throughout vessel if there is any significant temp change in any locations in tun.
Note: if you are going to batch sparge, your second batch should only need fifteen minutes to mash. Obviously, you can't get thirty minutes before runoff for that one. Just stir at water add time, put lid on and leave it.
