Default Chocolate Hazlenut Stout

My husband and i went to our LHBS to pick up some supplies for our next IPA and happened to be fortunate enough to be there at just the right time to sample an amazing chocolate Hazlenut stout. Does anyone have a recipe? This beer was SO good it would have made the perfect float. There were no over roasted flavors and the head was nice and creamy. A truely yummy beer that I would like to replicate!
Thanks in advance for your help.

1 lb. munich
1 lb. crystal 40L
1 lb. crystal 75L
.5 lb. carapils
.75 lb. chocolate
.5 lb. black patent

hops:
1 oz. kent goldings 60 mins

8 oz. unsweetened cocoa powder (last few mins of boil)

1 tab whirfloc (last 20 mins of boil)

1 pkg 25 ml hazelnut flavoring at bottling

4 oz corn sugar for priming

All grains can be steeped for 30 minutes from 150-170, remove grain bag, add dme and lme.

Have fun, and ask away if needed.

Made this one back in March and it is yummmmy!!! It is best about 3-4 months later.
